PAC Engagement Specialist

Join our client as a PAC Engagement Specialist to help post-acute providers adopt and use a critical engagement platform that improves care transitions and patient outcomes. You’ll design outreach and training strategies across SNFs, LTACs, home health, inpatient rehab and hospice networks and directly influence measurable engagement gains.

Compensation: Up to $31.50/hr — DOE, long term contract opportunity 

What You’ll Be Doing:

• Analyze user engagement data across post-acute networks and identify gaps.
• Design and execute outreach strategies to increase platform adoption.
• Coordinate and deliver training for large user groups.
• Capture user feedback and summarize product/process improvement opportunities.
• Track engagement metrics, produce regular reports, and recommend actions.
• Collaborate with Client Success, Network Operations, Implementations, and Training.

What You’ll Need to Be Considered:

• Experience working in post-acute care settings
• Demonstrated success in provider engagement and resolving adoption barriers.
• Ability to drive measurable improvements in user engagement scores.
• Familiarity with reimbursement/credentialing or CMS/Medicare/Medicaid concepts (preferred).
• Bachelor’s degree (preferred)
